You will need to provide the student’s banner ID, award amount, payout period and choose the proper scholarship fund in order to submit a nomination. This form limits each submission to 20 students for the same scholarship fund. You can submit multiple submissions as necessary. You will also be able to see your previous submissions by signing in.
